The Chronic Disease Management Plan can be accessed through your GP.  It entitles your child to up to 5 sessions per calendar year with an allied health professional.  To be eligible for this plan, your child's medical condition must be considered chronic, meaning it has been present for at least 6 months and they have needs in more than one area.  They must also be receiving care from at least two other health professionals.
